Medication Working?

I can a fantastic day. Sure, I got suddenly and abnormally tired sometimes... but I got through it. My pain was so much better! I hope it was not just chance. I hope the increased medication is working.

Besides slow increases beginning four hours after each pill, the only trouble I had was when I tried to sit on a metal folding chair, and my leg said, "You cannot be serious."

Sensory:
-Pins and needles: 3
-Tingles: 0
-Dysesthetic pain: 3, except for 6 when I sat on that chair for a few minutes. Sometimes it was 0 or 1!
-Paresthesias: 1, buzzy buzzes
-Numbness: 4, one weird new numb area was the arch of my foot. Well, feet.
-Vision: 2
-Nerve pain: 5
-Hearing: 0

-Walking: 2
-Sore muscles: 2

Long Day

Around 10:00 last night, I found out that I was giving a presentation at 10:00 am today. Yikes! And that I would have half an hour for what is usually a two-hour presentation. Double yikes. So I stayed up until midnight editing my powerpoint.

Nevertheless, I had a great day. I had very little pain in the morning, but then it came back. My energy level was decent; I did not hit a wall until around 6:00 pm. Just vegging since then--ah the exciting Friday night of a single woman.

Here is the crazy Catch 22 I am in right now: If I sit, my dysesthetic pain gets bad in my whole left leg/foot. If I stand or walk, the dysesthetic pain and pins and needles in both of my feet get bad.

I increased the gabapentin by 300 mg yesterday and today. Tomorrow and Sunday 600 mg increase... etc. No change so far, unless you count the good morning. I suppose that was just chance.

Oh! I got another "You look so good" last night. MS sucks.

Yesterday a friend advised me to have a story ready in case someone from work sees me using the loser scooter at Target or using my cane. Hm. Good thinking.

-Sleep: 6, because I got less than six hours--but I slept DEAD. Did not wake once.
-Bladder: 1

Emotions: I am too tired to remember.

End to the Pain?

I read a little more in the MS and feelings book. It made the point that feelings do not vanish; they only hibernate. Hm.

Dr. Rob said I can increase my gabapentin from 1800 mg/day to 2700 mg/day. Woohoo! Hopefully it helps. He said to give it two weeks; if it is not better, I need to go see him.

More Gabapentin, Please

Tomorrow I would like to call Dr. Rob and ask if he would not mind me increasing my gabapentin dosage. I had a relatively high-pain afternoon, and it was getting worse this evening. Just like yesterday. I was gearing up for another horrible evening.

My roommate gave me my bedtime gabapentin pill with my dinnertime pills at 7:00, and I popped it right into my mouth. I had taken my afternoon pill at 3:00, so it was early... but I was in pain and it seemed worth a try.

About an hour later, I was feeling much better. I had a low-pain evening. I suppose it could be chance, but I would like to give it a try.

Emotions: Random tears this afternoon. I was tired and in pain, and my body felt overstimulated. But still... tears? I was feeling overwhelmed, thinking I HAVE MS AND IT IS NOT GOING AWAY. I do not know why I am having so much trouble with the renewal of my diagnosis. Isn't this what I wanted? I certainly did not want lupus or NMO. I did not want MS, but I also did not want to be kicked into limbo land. I need treatment. Emotions are unpredictable.

Homework

Although I am only halfway done with the first book I picked up on MS and cognition, I am now reading a book about MS and feelings. It was written by a counselor.

There are some homework assignments in the book, and this is the first one. I am supposed to answer these questions in a notebook or journal and discuss them with my significant other. My what? Whatever, I do have a journal.

1. When you were first diagnosed with MS, how did you feel?

Anxious and scared. Mostly anxious. I was so worried about not being able to work. I worried that I would be in a wheelchair. I worried that I wouldn't be able to do things I love, like skiing.

2. Have your feelings changed since you first heard the news?

Yes. I am less anxious. I am more lonely. Maybe more scared. I still worry that I will not be able to do things I love or to work. The work thing is more about my cognitive problems than physical, though. Pain weighs heavily in my mind lately. If I feel this way for the rest of my life, I will scream. I also feel angry lately. I am not sure why I am angry or if it is directed at something.

3. What were some of your concerns or fears after initially being diagnosed?

How much longer will I be able to work? What treatments are available? What will I do if I can't work but I need these expensive medications? Which medications will help me? Is my doctor skilled? Is she right? Will she be able to treat me and help me get better?

4. What questions did you have?

What causes MS? What are the symptoms? And the worried questions above. Lots of questions about money and medical logistics. And questions about the disease itself.

5. Did you want to talk with others about it?

Yes! I wanted to talk with many people about it. I became more social than ever.

6. How did others respond to learning that you had MS?

Different people responded differently. I had just moved, so I was in the unusual position of making friends while going through the diagnostic process. My new friends did not know me before MS--the "real" me. Obviously they still don't. They responded well, offering rides and company at doctor appointments. That is all over though. MS is an exhausting, daily battle... Who would want to stay in it if they didn't have to?

I have not told my mom. She will tell everyone, and it will get back to people I work with. I cannot trust her to keep it to herself, so I cannot tell her.

My dad took it fine. He wrote me a prescription when I needed it. He questioned it at first, but not in a way that conveyed disbelief of my experience.

Some people who have known me for a long time have avoided it, and some have expressed shock and sorrow.

My best friend, whom I have not seen in about three years, was angry. Shocked. Sad. She already knew about MS (unlike me), so she knew how bad it could be. Actually, I do not think she did know how bad it could be. I now feel that severe pain is the worst outcome of any experience, and I know that people are unaware of the degree of pain possible with MS. But this friend knew enough.

My sister has distanced herself from it and from me. She does not ask questions, and she responds noncommittally to comments I make.

My other best friend did the denial thing, I think. She "forgets" that I have it, expecting me to do all the things I normally do for her. I am not a big whiner, telling her how much pain I am in all the time. I told her a few times--I think that should be enough. My MS has not gone away.

A good friend from work responded very well. She makes sympathetic comments and notices when I look sick. (Although I did not think I look sick.) She is great for emotional support.

So there you go. I did my homework.

Weather

By the way, it rained yesterday and today. Still wondering about the barometer and how it affects me. Is it just chance that two of my most painful days have been humid and rainy? Maybe. I suppose time will tell.

Officially Diagnosed...

...Again.

I spoke with Dr. Harry today. He supported the MS diagnosis given to me (twice, lol) by Dr. Idiot Gabby.

My spine MRI was clear--no visible lesions. My Vitamin D level was 50, which is right in the middle of where it should be. My calcium level was fine; no increased risk for kidney stones for now (whew). I tested positive for the JC virus, which means that I am at increased risk for a fatal brain infection if I choose to use a medication called Tysabri.

The tests for copper deficiency, Sjogren's syndrome, neuromyelitis optica, and lupus were all negative. What a relief. Gee, it is only MS.

So I can keep my screening appointment with Dr. Rick next week. If I do not qualify for the study, I can speak with Dr. Harry about getting on a disease-modifying drug.

But that means I will really be injecting myself. This is suddenly more real. I have mixed feelings--let's save that for the emotions section, hm?

Meanwhile, I feel like I might be coming down with something. Hopefully it is not the explosive diarrhea one of my students had yesterday!

Sensory:
-Pins and needles: 4? This, dysesthesia, and numbness all blur together. It is hard to separate them.
-Tingles: 0? The pain blurs other feelings.
-Dysesthetic pain: 8. Seriously. I should have known when it was already a 4 when I woke up that today would be a doozy. It was up and down, but this evening has been absolutely out of control.
-Paresthesias: 1, foot buzzing, which has happened a few times each day lately
-Numbness: 5, partially numb legs and feet, plus tongue issues. This evening, I have been lying on my stomach, trying to give my sciatic nerves a break. Well, my arms keep getting numb, since I am bearing weight on them. Out of the frying pan and into the fire.
-Vision: 2, still on the blurry side, plus I am very light sensitive. 
-Nerve pain: 7
-Hearing: 0

Motor:
-Walking: 1
-Sore muscles: 3
-Fine motor: 3
-Weak muscles: 4
-Spasticity: 4, the regular tightness and some muscle twitches in my right glute

Cognitive:
-Language processing: 2, mostly having problems with written language

General:
-Fatigue: 5
-Balance: 2
-Sleep: 4, had trouble sleeping again--bad dreams, waking up scared
-Bladder: 2, which is so discouraging

Emotions: I am relieved that I will get treatment. I am relieved that I am being believed. I am apprehensive about giving myself injections. Forever. The rest of my life! That is overwhelming. I am sad that I have MS. I do not think it has fully hit me. Maybe people are right that it takes a year to process and accept the diagnosis.

This cannot be my reality.

Come On, Dr. Harry

So frustrated with Dr. Harry! I called this morning, and his secretary had no clue. Finally Dr. Harry called me back... and I missed the call! He left a message saying that he was calling to discuss my lab results. I picked up my phone 20 minutes later and kicked myself for not having it on me. I called right back, but I have not heard back from him. You had better believe that phone will be glued to me tomorrow.

I am pretty ticked that he had my labs and did not call me until I bugged him.

-Dysesthetic pain: 6, whole left leg and foot, from my butt on down
-Paresthesias: 1, foot buzzing, which has happened a few times each day lately

-Language processing: 3, getting overwhelmed by a lot of language coming at me. And I got a little frustrated when my words would not come out correctly and my language was disorganized. A little frustrated. Not a lot. I know that my poor brain cannot help it.

-Attention: 4, I am taking longer to realize that I spaced out or focused on something else
-Confusion: 2

General:
-Fatigue: 4
-Balance: 2, I got up and almost fell over! Luckily I fell into a wall, so harm done.

-Bladder: 1, yes, misbehaving again. :(

Emotions: Got a bit angry and frustrated today. Meh. Not terrible.

I also felt gratitude and love, to and from a somewhat unlikely source. Remember when I mentioned the experience with "Little Doll?" I ended up telling Little Doll's mother that I have MS. She is a nurse and knows exactly what this means for me. Well, she has been so kind about it since then. We have not discussed it much--professional boundaries--but she has asked how I am feeling, helped me up when I have difficulty getting off the floor for the 1,000th time at preschool (it is a parent-supported preschool), etc. Those little things are always touching. Today I did a home visit, and she gave me a big hug as I was leaving.

So remember how much your hugs and kind words can brighten someone's day. This woman's certainly did for me today!

Church Is Good, Numbness Is Bad

I forgot to mention that I called Dr. Harry's office Thursday and Friday. Ready to run down there and hold a sit in to get the results of my bloodwork and MRI. I never did read the MRI myself, but I am planning to tomorrow. If I can remember. Which I probably cannot.

Today I went to church. Yea! I like church.

I cannot believe that I am still interpreting most of the meetings. Crazy. I am not interpreting in the community, except for band and orchestra performances (they pay me to announce the names of songs and then sit there--what a gig!), which is because I totally suck at interpreting.

Sensory:
-Pins and needles: 1, haha I was about to rate this 0 until I checked in with my body... yeah, I have them right now in my left foot
-Tingles: 0
-Dysesthetic pain: 3
-Paresthesias: 1, some itches that I am pretty sure are not itching from histamine
-Numbness: 4, left foot/leg off and on--well, on when sitting, even on a well-padded chair. Thigh front problems just hit now. And I had mild tongue sensation problems again today. Losing feeling in my mouth no es muy bueno. (Yes, I totally pretended that I speak Spanish?)
-Vision: 2
-Nerve pain: 5
-Hearing: 1, darned tinnitus again this pm

Motor:
-Walking: 2
-Sore muscles: 4
-Fine motor: 3--tried to play the organ, boo. And piano, mini boo. It seems like the more I practice, the worse I get. My motor skills decline quickly. And the organ is just so much harder than it used to be.
-Weak muscles: 5, it seems like I downgrade this on days that I do not do anything strength-related, ha
-Spasticity: 4

Cognitive:
-Language processing: 3, my auditory processing suuuucks. Yeah, I had trouble understanding every single person who shared their testimony today. So?? Hm, but Sunday School was not bad. I had to focus and listen, but that was more an attention thing. And RS was fine, except again needing to focus. I suppose the fact that Sacrament meeting uses a sound system and I am two feet from the actual speaker during SS/RS might make a different. Yeah.
-Memory: 2
-Attention: 3, see above... sigh
-Confusion: 1

General:
-Fatigue: 6, I just wanted to curl up in a ball at church today. My body was begging to lie down. I vegged out for a while after church, and then I went back after the other ward was done to practice the organ. I played for about 45 minutes, which is unreasonable in my current condition, but I was in musician's trance. Like stitcher's trance--once you start, you can't stop! ;) Well, I paid for it afterward.
-Balance: 1
-Sleep: 4, went to bed a bit late and got up a bit early--not by choice! I wanted to hit the 8:00 am choir practice today. Um, I guess that is by choice. I mean, not by force, lol. Choir was totally worth it!
-Bladder: 0
-Proprioception: 1

Emotions: Someone at church asked me how I was doing while I was standing up to stretch my legs between meetings. I said that I was doing okay, that I was just trying to feel my leg again. I wasn't happy about it, but I wasn't terribly bothered. Gee, darn.

Later I went huh? I do not think I appreciated the magnitude of my response: "Trying to feel my leg again." Who says that????

Sometimes I think my emotions are blunted, or maybe I am like an abuse victim who has a new (horrible) normal. Maybe it is just adaptability. Maybe it is distance between my consciousness and the situation. Maybe it is a way to get through the day without crying! 

Sadly, I did not mind the numbness I was feeling, because it was the cold numbness, which is way better than the mixed numbness and pins/needles. The cold numbness feels like ice water is being poured through me. It was not really painful, so I was feeling lucky. Yeah, definitely a new normal if a half-numb leg is "lucky."

Spasticity

I was lying on the floor, on my stomach, trying to lessen the pain. I was really struggling with spasticity, and I told my sister to check out my rock-hard calves. Well, half rock hard--just the lateral split of the calf muscle. She could feel the difference between the lateral and medial branches of the gastrocnemius.

Now if only I could get spasticity in my abs...

This Disease Is Unbelievable

Today I tried to spend less time sitting. I laid on my stomach when I was reading my new MS book--that helped with my lower limb sensory problems--pins and needles, dysesthetic pain, and numbness. But I cannot exactly lie on my stomach on a bed all day.

**I had published this earlier, around 5:30, but now (8:30) I have to upgrade the pain and spasticity ratings. Evenings are generally worse.**

Sensory:
-Pins and needles: 1
-Tingles: 1
-Dysesthetic pain: 5, front of both thighs (this has happened occasionally in the last two days or so), as well as left foot and leg, also right leg this evening (down to calf)
-Paresthesias: 0
-Numbness: 4, having more problems with my left foot. It feels like cold numbness blanketing part of my foot--more lateral. Also partial numbness in my tongue and lips, which was happening yesterday. And the day before? Hard to make a good report, because I am forgetful. This is the right half of my lips--uh... lower only? Now it is getting better, so it is hard to say.
-Vision: 2, definitely still blurry, glowy around light colors, blah blah blah
-Nerve pain: 7, left leg and even some in right leg
-Hearing: 0, rockin'

Motor:
-Walking: 4, It was pretty good this morning, but now it stinks. My legs are just not listening to me.
-Sore muscles: 5--totally from spasticity
-Fine motor: 2, having trouble typing
-Weak muscles: 3
-Spasticity: 6

Cognitive:
-Language processing: 3, word finding, organization, getting a thought out before part of it is lost--losing quite a few thoughts today

-Sleep: 6, was out with friends last night and woke up at 5:00--noise? Then I couldn't sleep because of GERD and dysesthetic pain in my left leg.

Emotions: I felt some anger at MS today. And fear when I was reading the book about cognitive problems with MS. This disease is unbelievable. It hits every part of the body!

Doubt

I need to stop second guessing and doubting myself. I know what I am dealing with. Cognition and lower limb sensory issues are my main barriers to normal life lately. Fatigue has been less of an issue this week, although I certainly do not have the energy level I normally do.

These problems are all invisible. Isn't that delightful? The doctors have to take my word for it.

If I do not project confidence in my experiences, my symptoms, how can I expect the doctors to take me seriously?

That question is easily answered, but that does not mean that the problem is easily remedied.

You know, I am so doubtful of myself and my diagnosis that I have not changed the title of my blog.

Good Happenings

I had a good day in many ways. Not so much physically but emotionally.

1. I had a great visit with my shrink. Many tears fell, which is typical for me. I did some thinking, and I have more to do--if only I can remember what we discussed! My poor brain. Speaking of which...

2. Yesterday a book about cognitive challenges with MS arrived. I have been devouring it today. It is a little scary that the author left his career within two years of developing MS, completely due to cognitive issues. I would really like a complete neuropsychological evaluation.

3. Today I got a newsletter from the state MS chapter. It had some news, including the discovery of 29 news MS genes... and they confirmed 23 already-identified genes. Wow. No idea. But what really caught my eye was the list of support and fitness group meetings. I called the woman for my local support group, Maby. There is not a regular meeting this month, but they are having a holiday party on the 21st. I am going to go. I meant to call the fitness group person, but I forgot.

Sitting and MS Are Unfortunately Not Friends

I had an alright day, except for quite a bit of pain this evening. It began in the afternoon but became worse in the evening. I was sitting on a chair that was apparently not padded enough for my sciatic nerve's liking--picky! So my left leg and foot kept going semi-numb, pins and needles, and pain. Whatever.

-Dysesthetic pain: 4
-Paresthesias: 0
-Numbness: 4
-Vision: 2? I haven't paid attention to whether it still is. I know that I had a ton of floaters this morning, which are pretty much gone now.

-Fine motor: 2, blech, crappy handwriting. My fingers seem to have forgotten how to hold a pen.

Great Day!

I enjoyed going to a church social tonight! I had a good day today, except for my uncooperative brain. I had pain, as always, but it was not bad.

Sensory:

-Tingles: 1, left hand and... uh.. that's all I remember
-Dysesthetic pain: 2
-Paresthesias: 0?
-Numbness: 2, one thing I have noticed lately is that my phone responds before I touch it. It responds right before I touch it, in fact. Duh, I think I must have already touching it when it responds, but I am not feeling it--my phone is more sensitive to touch than my fingertips.
-Vision: 2
-Nerve pain: 4
-Hearing: 1, tinnitus this morning

Motor:
-Walking: 3
-Sore muscles: 1
-Fine motor: 1? Didn't need to write. But my typing is not bad.

-Language processing: 3, I had an embarrassing moment where I completely misunderstood something during a meeting and spoke up to disagree with what someone said... ugh. Also, I am usually a very quick thinker and able to respond and counter arguments. I let a major one go right by me! Honestly, I did not participate in the meeting as much as a could have normally. And I was taking minutes for the meeting and struggling with organizing and summarizing. Also having trouble organizing and expressing my thoughts during conversation this evening. It is so ironic.

-Attention: 2, I totally zoned out or focused on something else during conversations several times today.

Fuzzy Hearing

I forgot to mention that I had very mild tinnitus yesterday, and it is worse today. Still mild, but noticeably worse than it was.

It sounds like fuzz... whitish noise... bugs in the summer...
